Literature Highlight: Erythromycin versus Neomycin in the Treatment of Hepatic Encephalopathy

ثبت نشده
چکیده

Hepatic encephalopathy (HE) is a common complication in patients with hepatic cirrhosis. Ammonia from intestinal overgrowth with bacteria is the main cause of HE symptoms. Treatment of intestinal bacterial overgrowth with antibiotics that have low gastrointestinal absorption demonstrates the most benefit. However, increasing gastrointestinal motility is another integral part of treatment and currently used antibiotics, such as neomycin, lack this ability. To date, no treatment modalities treat bacterial overgrowth and increase motility. Erythromycin is an oral antibiotic that has been used as a pro-kinetic agent and has the potential to work on both aspects of HE.

Erythromycin versus neomycin in the treatment of hepatic encephalopathy in cirrhosis: a randomized double-blind study

BACKGROUND Hepatic encephalopathy (HE) is a severe complication in patients with hepatic cirrhosis, which causes numerous hospital admissions and deaths. Antibiotics are the best options in HE treatment, but head-to-head comparisons between these drugs are scarce. Erythromycin combines the antimicrobial effect and prokinetic properties in the same drug, but it has never been used in HE treatmen...

Treatment of hepatic encephalopathy with metronidazole.

Neomycin, an antibiotic which is primarily active against the aerobic gut flora and hence reduces the endogenous production of ammonia, is a well-recognised form of treatment for acute or acute on chronic hepatic encephalopathy. This study suggests that metronidazole may be a useful alternative or even adjunctive treatment for such patients. Theoretical and practical justifications for the use ...

Relative efficacy of neomycin and ampicillin against urease producing organisms.

The efficacy of neomycin in hepatic encephalopathy was highlighted3 as early as in 1957. The azotemia associated with aome fonru; of severe liver diseaseS militates against the use of neomycin in these circumstances6-8. The usefulness of ampicillin has been reported and the antibiotic has been suggested to be a satisfactory substitute9•1o. However, no study has directly compared the efficacy of...

Evaluation of the role of TLR4 in endotoxin-induced hepatic encephalopathy in rats with biliary cirrhosis

Background: Hepatic encephalopathy is defined as a neuropsychiatric brain dysfunction in acute or chronic liver failure. Infection and inflammation have crucial role in its pathophysiology. The purpose of our study was to demonstrate the relationship between toll-like receptor 4 (TLR4) expression and the encephalopathy induced with endotoxin in biliary cirrhotic rats. Methods: The present stud...

Current trends in the treatment of hepatic encephalopathy

Hepatic encephalopathy (HE) is a common reversible neuropsychiatric syndrome associated with chronic and acute liver dysfunction and significant morbidity and mortality. Although a clear pathogenesis is yet to be determined, elevated ammonia in the serum and central nervous system are the mainstay for pathogenesis and treatment. Management includes early diagnosis and prompt treatment of precip...
